Health Intake Form: All clients must review the following policies outlined below, then complete the digital Health Intake Form located at the end of this page before your first visit.

​

Late arrivals/cancellations/“no-show” appointments: Please arrive on time for your appointment, and notify us if you anticipate a delay. We will do our best to accommodate the appointment within the allotted service time previously scheduled.​ If you need to cancel, please do so outside of 48 hours. In the event that you are unable to use the online scheduling system, please contact us directly. Full payment of appointment is required for no-call, no-show or last-minute cancellations.

Snow Days: Please be sure your contact information is up-to-date so that we may stay connected in the event of inclement weather. If driving conditions are unsafe, we will likely close for the day.

Payment: We accept all major credit cards, cash, or Venmo as forms of payment. Full payment is due at the time of service. If you are paying with a gift certificate, you must have the gift certificate information readily available. Kindly note that gift certificates are only redeemable with the practitioner from whom they were purchased.

Sickness: Bodywork and massage are not appropriate care for infectious/contagious diseases or sickness. Please cancel your appointment as soon as you are aware of your illness, or contact us directly if you are unsure if it’s appropriate. Please cancel your appointment if you are unwell.

Regarding Traditional Thai Bodywork with Sara-Lee:

​

Clothing: Please wear (or bring) comfortable, loose-fitting clothing; no shorts or skirts; layering may be appropriate. There are a few techniques that require the removal of clothing; in these instances, draping will be used. During a typical session, liniments, balms, oils, and herbs may be applied to the body, potentially staining fabric. If this is a concern, please notify us immediately.

Cupping/Scraping/Tok Sen: In some instances, traditional fire cupping, or “cupping” and/or “scraping” therapy (aka graston or gua sha), or tok sen, may be used as part of your treatment plan. These therapies may include risks of tenderness, bruising, redness, and, in many instances, discoloration of the skin, which may last up to 2 weeks. You may refuse these therapies at any time.

Codes of conduct and service: We always treat all patients with the utmost respect and care. Our goal is to provide excellent customer service. However, we reserve the right to refuse service to anyone at any time for any reason. We are dedicated to providing a safe, healthy, healing environment for all. Any level of misconduct or harassment will not be tolerated.

 

Acknowledgment of Consent for Care and Liability Release (signature required for “first time” patients in the form below): Your signature below ("Patient Health Intake") confirms the accuracy of the provided information and your agreement to notify your massage therapy practitioner of any updates to your health history. By signing the Patient Health Intake form, you will be recognized as a client of d/b/a Maine Healing Arts, for the treatment of bodywork and massage therapy. Bodywork and massage are not intended to be a substitute for professional medical care and are not meant to cure, diagnose, or treat any medical conditions. On rare occasions, clients may exhibit adverse reactions to bodywork and massage; symptoms may include: dizziness, muscle soreness, bruising, and allergic reactions to herbal products, among others. You are in complete control of your bodywork and massage session. Should you experience any of these symptoms or feel any discomfort, please notify your massage therapist for session modifications or termination. Your signature on the “Patient Health Intake” form below signifies your agreement to not hold your massage therapist liable for any adverse effects you may experience during or after your bodywork and massage therapy session. Traditional Thai Bodywork and massage sometimes require close contact of client and practitioner and may involve more client/practitioner contact than many people have experienced during other types of massage sessions. If you feel uncomfortable at any time for any reason, please inform your massage therapist immediately so that direct action may be taken to remedy the situation or discontinue the session, according to your preference. By signing this form, you also understand that any illicit, suggestive, or sexual remarks or advances will result in immediate termination of the session.
